Each Incident form has a section labeled Root Cause Analysis. This provides the ability to record and capture the root cause of any incident.
Simply expand the folded region and enter in Root Cause and Required Changes into the expandable text areas. Please note that this is an example in its simplest form - your administrator may have altered or added new fields to this area.
